(5) That the Plaintiff is informed and believes that he has tried to get a Social Security card for the child and has been told that since his name is not on the birth certificate that he cannot get one for the child until the birth certificate is amended to reflect that the Plaintiff is the father of said child.

( 6) That Plaintiff is informed and believes that the birth certificate of the minor child, L.S.L., should be ordered to be amended to reflect that he is the father of L.S.L.

(7) That the Plaintiff is also informed and believes the minor child’s last name should be changed to Wright.
