PUBLIC NOTICE FOR INVOLUNTARY TERMINATION OF MEDICARE / MEDICAID PROVIDER AGREEMENT Notice is hereby given that effective May 14, 2015, the agreement between South Carolina Medical Endoscopy Center, 1735 Taylor Street, Columbia, SC 29201 and the Secretary of Health and Human Services, as provider of Ambulatory Surgical Center (ASC) Services in the Health Insurance for the Aged and Disabled Program (Medicare) is to be terminated. South Carolina Medical Endoscopy Center does not meet the following conditions of coverage: 42 CFC 416.41 – Governing Body 42 CFC 416.42 – Surgical Services 42 CFC 416.46 - Nursing Services 42 CFC 416.51 – Infection Control The Centers for Medicare and Medicaid Services has determined that South Carolina Medical Endoscopy Center is not in compliance with the conditions of Coverage. Therefore, the Medicare program will not make payment for ASC services to patients who are admitted after May 14, 2015.
